The U.S. Department of Health and Human Services is awarding Minnesota nearly $5 million to stem chronic diseases and promote healthier communities.
Through the federal health care law's Community Transformation grants, the Minnesota Department of Health will receive $3.6 million and Hennepin County will receive nearly $1.2 million.
The money will fund programs aimed at targeting the root causes of chronic illness such as tobacco use, unhealthy diets and lack of exercise.
